What are Hospice-Veteran Partnerships? Hospice-Veteran Partnerships (HVP) are coalitions of Department of Veterans Affairs (VA) facilities, community hospices, State Hospice Organizations, and others working together to ensure that excellent care at the end of life is available for our nation’s Veterans and their families. These coalitions may be community-based or statewide and may function independently or within an existing structure. HVP is a national program of the Department of Veterans Affairs (VA) Hospice and Palliative Care Initiative.

Uncle Chuck
"Uncle Chuck retired from the Navy, and in his eyes, his service to our country helped mold him and later define him. Having seen the horrors of war and lived a life we can only imagine it was extremely comforting to know that the hospice company was trained and prepared to help him with these psychological battles. They understood him. They comforted him. They worked diligently to put a photo album together of his time in the service. This helped him reconcile issues he had held on to for decades. He died peacefully surrounded by family. The photo album was then presented to our family with special notes that Uncle Chuck wanted to share with us. This was an amazing gift of love and forgiveness." Michael J.
